K-Registration Packet Request Form

To download the Kindergarten Registration Packet complete the form below.

You MUST return the following forms to your school office:

1. Registration Sheet
2. Affidavit of Residency Form
3. Kindergarten Registration Supplement
4. Original Birth Verification (Certificate, hospital record, baptismal certificate)
5. Original Immunization Record from Physician/Health Agency
6. Residence Verification (3 forms are required, such as a SCE bill, 1 other bill, tax documents, DMV registration, escrow/lease agreement, cancelled check)

Transitional Kindergarten Information
If your child will turn 5 between Oct. 2 and Dec. 2, you may qualify for Transitional Kindergarten.

IMPORTANT: Students will only be registered once completed packets are received at the school of attendance. Only completed packets will be accepted - no exceptions.
